Mortuary Shipping Guidelines

Whenever a funeral home is preparing a body for shipment, strict guidelines must be adhered to. The family must provide the name of the funeral home receiving the body.
The funeral home in charge of shipping the body must provide the following documents:
  • Communicable disease affadavit
  • Embalmers affadavit
  • At least two certified copies of the death certificate
  • The burial/transfer permit
  • Authorization forms
All documents must be notarized and the passport of the deceased must be shipped. A complete flight itinerary must be provided and consideration for consulate inspection must be made. Translations of all documents must be provided by the funeral home, depending on the receiving country.

The requirements for cremated remains are similar when shipping. Once the body is cremated, it must be packed in a wooden crate. This crate must also contain:
  • Two certified copies of the death certificate
  • Two certified copies of the cremation certificate
  • The non-contraband affadavit
